HEALTH & DENTAL PLAN

The LPSA Health, Dental, Mental Health & Wellness and Legal Care Program fees are automatically assessed to all eligible students. A Student Referendum approved the Student Benefit Plan Fees and are an admission requirement at Lethbridge Polytechnic through your membership in the Students’ Association. The cost of the plans is included in your institutional fees provided you meet the eligibility requirement of enrollment in a minimum of 6 credits.
The www.mystudentplan.ca/lethbridge website provides detailed information covering Enrollment, Benefits, Claims, Savings, and Online Applications to add family and optout and contact information for inquiries.
Eligible students receive 12 months of coverage starting the first day of the month your program begins. The cost is included in your tuition.
New eligible students are added to the health to the benefit plans approximately 60 days from the start date of your program. Please keep receipts for eligible expenses incurred during this time for submission to the insurance carrier once the enrollment process is complete. You will receive an email from the LPSA to your Lethbridge Polytechnic email address advising the benefit plans are active and at that time you can download the mystudentplan Benefits App and register to access your benefit plan cards and submit any receipts online for receipts held during the validation period.
Online/Distance Learners – are included on the benefit plans provided the eligibility requirement of enrollment in a minimum of 6 credits is met.
Returning Eligible Students – may continue to access the benefit plans without disruption by using the Benefits App.
*Opt-In – if you have previously attended Lethbridge Polytechnic (College) and Opted Out, you can re-enroll by completing the online Opt-In Application and it will be received at the Benefit Plan Office to be approved and once approved you will receive an approval email with the fee amount for the 12 months of coverage required and the link to make payment by the term deadline. Going forward the fees will appear again in with your tuition.
Family Add-On – coverage is available to add your spouse/partner and/or dependent children by completing the online Family Add-On Application found on the benefit plan website. The benefit plan office will receive the application to be approved, once approved you will receive to the email used to apply, the approval email and included in the email will be the link to make payment for the 12 months of coverage fee and the deadline date the payment is required. Students must be enrolled in the health and dental plans to add family.
FAMILY ADD-ON MUST BE APPLIED FOR EACH BENEFIT PLAN YEAR PRIOR TO THE ENROLLMENT PERIOD DEADLINE.
Opt-Out – Students with *proof of comparable Health and Dental Coverage can Opt-out of the plans before the Opt-out, extended drop date deadline in the term or on the anniversary of the term of enrollment by completing the online Opt-Out Application found on the www.mystudentplan.ca/lethbridge website. Please Note the Terms and Conditions you must agree to when completing the one-time Opt-Out. There are no exceptions if the posted deadline is missed. Please keep your confirmation email when opting out as your proof of opt-out.
* Provincial Health care coverage, and the Canadian Dental Care Plan (CDCP) DO NOT qualify as proof of coverage.
